In the fourth report in a series by the Insitute of Medicine, the report concluded medication errors, nationwide and in Oregon, harm 1.5 million people and kill several thousand each year. The annual cost to our nation is at least $3.5 billion. The Institute of Medicine is one of the nation's most prestigious medical advisory organizations and has brought attention to the enormous health and financial burdens brought about by medical errors.
